WSIB makes available a Worker’s Exposure Incident Form that is for voluntary use in the event a worker has an unplanned workplace exposure to various dangerous substances, including those that are infectious.
As more COVID-19 cases appear in the workplace, successful claims will have to be able to track exposure and show that the workplace was a significant contributing factor to the illness. If you believe that you have been exposed and has not experienced symptoms yet, you should complete the attached form, fax or mail a copy to WSIB, and keep a copy for your records.
